Problem

Korsana Biosciences is addressing the difficulty of treating neurodegenerative disease with large-molecule drugs. The blood-brain barrier sharply restricts entry into the central nervous system, so traditional biologics deliver little drug to the brain; increasing doses to compensate can reduce safety and efficacy. The immediate use case is Alzheimer’s disease, where the patient and caregiver burden is severe, long-term healthcare costs are projected to exceed $1 trillion by 2050, and existing disease-modifying therapies have only modest efficacy alongside ARIA safety warnings, frequent intravenous infusions, and burdensome MRI monitoring.

Product / Service

Korsana is developing therapeutics rather than selling a currently marketed service. Its core technology, Therapeutic Targeting, or THETA™, is a blood-brain-barrier shuttle platform developed with Paragon Therapeutics that uses transferrin-receptor biology and antibody Fc engineering to transport large molecules into the brain. The lead candidate, KRSA-028, is an investigational anti-amyloid-beta monoclonal antibody designed to bind plaque-enriched pyroglutamate amyloid beta and clear plaques while improving brain exposure, safety, dosing convenience, and compliance.

KRSA-028 is designed for low-volume subcutaneous administration, potentially avoiding high-dose infusions and reducing the need for infusion-center visits. Korsana says its engineering is intended to preserve plaque-clearing activity while reducing unwanted immune effects and risks such as ARIA and hematologic adverse events, but these benefits remain investigational and must be demonstrated clinically. The company is also pursuing additional, undisclosed THETA-enabled neurodegenerative-disease programs.

Market

Korsana competes in the Alzheimer’s disease and broader neurodegenerative-disease therapeutics market, specifically the emerging market for brain-penetrant anti-amyloid antibodies. Its reference and competing approaches include approved Leqembi and Kisunla, Roche’s transferrin-receptor-linked trontinemab, Denali’s DNL921, AbbVie’s asset acquired from Aliada Therapeutics, and Lilly’s follow-on remternetug. The competitive opportunity is to improve on the efficacy, safety, administration, and monitoring burden of first-generation anti-amyloid therapies.

As of August 1, 2026, Korsana is best characterized as a well-funded, pre-commercial biotechnology company rather than a revenue-generating commercial-stage business. It was founded in 2024, raised $25 million in seed financing and $150 million in Series A financing, and had 17 full-time employees when it emerged from stealth. KRSA-028 was still described as preclinical or in discovery, with clinical development and 2027 safety and proof-of-concept readouts planned. Korsana also announced a proposed reverse merger with Cyclerion and approximately $380 million of financing, expected to close in the third quarter of 2026; the evidence gathered here does not confirm that the transaction had closed by August 1.
